After reviewing the Charter and the Regulation on Membership Fees of the Organization, I agree to the Regulation, support the goal of the Organization

To receive a membership card of the Organization, please send by e-mail: sci.guild@gmail.com

a file with a scanned individual identification / tax number,
file with scanned passport (first pages, registration),
photo for documents (JPEG file - resolution not less than 300 dpi),
a file with scanned document about education / academic degree/ academic title (if any) (in PDF):

a) certificate of secondary education;
b) diploma of vocational education and its supplement;
c) diploma of higher education and its supplement;
d) certificate of associate professor / doctor of science / researcher, etc.;
e) copies of other certificates, licenses, certificates that the applicant wishes to submit additionally
